-1- There are no experts on this board for you. That is you being the idiot. Note, I’m not saying you are an idiot. I’m saying trusting this board for your mental health decisions is ignorant. It’s idiotic.

-2- No one on this board has sufficient knowledge of your ongoing issues and mix of medications to provide high quality advice.

Stop using the board as a substitute for the advice of your doctors.

Presumably desperate has been on the forum some time. What I find odd is that desperate is employed, which suggests he must be able to function on a daily basis.
I know a few people with serious mental health problems. Two are listed as violent. One truly is violent. In the overwhelming majority of cases, the family stress and situation is ongoing.
I agree desperate requires support on a personal basis, face to face with the practitioner. However, in my experience that alone isn't enough. The person affected must have a clear aim to gradually improve and control the condition. In this case I sense desperate believes one person, doctor or therapist can solve his (or her) anxiety and depression. The doctor or therapist can help point the way but always - in my view - the patient alone has to make a firm decision to break the cycle.
Lastly, on this forum we should all tread carefully. I've known a few people with serious disorders and we need to be aware mental health affects many many people. My own mother, for example, had a serious breakdown and alcohol problem (caused by depression).
Yes, here, we are not able to substitute a good, trained mental health practitioner. We can be supportive of course.

Hope this helps you.
(1) Be positive about your chances to improve. Thousands of people and families suffer the stress of mental health.
(2) Therapy is a long and complex path. You need a flesh and blood psychologist to consult because forums are too impersonal.
(3) Understanding yourself is the key to improvement as is self acceptance. That is, learning to accept yourself as you are.
I recommend you read a few quotes by Jiddu Krishnamurti and think about his outlook.
(4) When you post on here, try and remember we can't solve issues you need to discuss with your doctor. It's far better if you join in on discussions so you relate better to members over time.
(5) One tip. Try to read books or just focus on something. Happiness often comes when you cease to seek it out.

Would it be true to say your thoughts "jump"? I get this impression. There's a term for it. Sometimes it's mild and in others the speech is very jumpy and so the train of thought jumps from topic to topic.
How is communication with the psychiatrist?
On the forum you tend to post very brief messages but it leads to very little two-way communication. This I understand very well. I didn't always structure my thoughts. On this forum I appear very structured but there was a time nobody could understand my communication. In fact, there was a time I didn't look at or notice the other person. I had to learn the process. On a forum for me it's easy but in actual conversation it's one way traffic, on a bad day.

Here, you responded reasonably to each point I made. That's good.

Jiddu Krishnamurti:

"Does self-knowledge come through search, through following someone else, through belonging to any particular organization, through reading books, and so on? After all, that is the main issue, is it not?, that as long as I do not understand myself, I have no basis for thought, and all my search will be in vain. I can escape into illusions, I can run away from contention, strife, struggle; I can worship another; I can look for my salvation through somebody else. But as long as I am ignorant of myself, as long as I am unaware of the total process of myself, I have no basis for thought, for affection, for action."
